They’ve basically given us 2 different ways to incorporate their service into your website. We can talk through the options to help give you a better understanding if necessary.

Option #1 is to link (with a button) to an external form like this:

Option #2 would be to embed the form on one of our pages. Hopefully I’d be able to edit it a bit to look a little better:
